Frequency Of Temporomandibular Joint Dysfunction With Clicking Symptom Due To Primary Molar Premature Loss In Children Aged 6-12 Years Old.

Temporomandibular joint dysfunction with clicking symptom is sound “click” when open and close the mouth. The purpose of this study was to obtain the description of frequency of temporomandibular joint dysfunction with clicking symptom caused by premature loss of primary molar teeth in children aged 6-12 years old at SDIT Imam Bukhari. The study was a descriptive survey, 136 students as samples were taken by purposive sampling technique. Premature loss of primary molar teeth examinated by checking up the irst and second primary molars upper and lower jaw using the hand mirror and explorer. Temporomandibular joint dysfunction examinated by palpation and using stethoscope. The result of this study showed that from 136 students having premature loss there are 49 students (36,03%) had temporomandibular joint dysfunction with clicking symptom. The conclusion of this study indicates that more than one-fourth students at SDIT Imam Bukhari with premature loss of primary molar teeth had temporomandibular joint dysfunction with clicking symptom.

Temporomandibular joint (TMJ) is a component of the stomatognati system that supports oral dental health which their components are closely interconnected. Posture is a habit that often do not realize that could cause an imbalance in the joints and muscles, which in turn causes pain. Inadequate body posture can occur on sleep position, lying down, sitting, walking, and other daily activities. Most people are not aware of the dangers of these habits. Dentists are expected to shed some light on the habits of this particular posture errors, which if not promptly removed, it will cause interference with stomatognati systems, such as joint and muscle pain, headache and neck. Complaints due to bad habits can be reduced significantly by eliminating the habit. So it is with the wrong posture habits. With improved posture and a series of exercises as well as the care, the health of TMJ can return to normal with minimal costs. The first step of clinical management of the temporomandibular disorders (TMD) is to distinguish the origin of problem, to classify into myogeneous and joint problems, which have been regarded to be an important process. Although the precise mechanisms for the symptoms or pathology of these conditions have not yet been clarified, muscular parafunction or hyperfunction has been regarded to play an important role in the etiology of TMD. Excess mechanical loading to the temporomandibular joint (TMJ) has been reported to be one of the possible causes for the development of osteoarthritis. These forces might be exerted through mandibular parafunctions like sleep bruxism or tooth contacting habit. For clinical control of sleep bruxism, a thick palatal appliance which fills the palate and does not cover the occlusal surface have been developed. In this review article, clinical implication of the relationship between occlusion, mastication and bruxism have also been discussed.

The electromyographic activity of the anterior temporal and right masseter muscles correlates positively with temporomandibular joint disorders. 10

Temporomandibular Disorders (TMD) is a disorder that is still controversial in terms of correlation between clinical signs & symptoms and age. Most studies were carried out on adult populations with little attention given to the elderly. The purpose of this study is to elucidate the frequency of TMD in elderly population and risk factors pertinent to it. This is a cross sectional study which included patients aged 60-91 of Geriatric Department of National Hospital Cipto Mangunkusumo, Jakarta, Indonesia. The temporomandibular joint (TMJ) was examined for clinical signs & symptoms. Data was analyzed using non parametric Chi Square test. Although there is no significant p value obtained, there is a clear tendency that occurence of TMD diminishes with age, more male elderly experience TMD and elderly experiencing up to 13 tooth loss might have higher risk for TMD. B. Patofisiologi Dislokasi Temporomandibular Joint (TMJ) Dislokasi dari temporomandibularjoint (TMJ) seringkali timbul dan disebabkan oleh hipermobilitas dari mandibula. Subluxation (dislokasi parsial dari sendi) menyebabkan pemindahan dari kondilus, biasanya tidak membutuhkan pengelolaan medis. Kondisi yang lebih serius timbul ketika kondilus mandibula bertranslasi ke anterior di depan articular eminence dan terkunci pada posisi itu. Dislokasi dapat terjadi secara unilateral atau bilateral dan dapat timbul secara spontan ketika mulut membuka secara lebar, seperti pada saat menguap, makan atau pada saat prosedur perawatan gigi. Dislokasi dari kondilus mandibula yang bertahan lebih dari beberapa detik biasanya akan menyebabkan sakit dan biasanya juga menimbulakn kejang otot parah.

There are a lot of experts that have written about TMJ dysfunction treatment. The data shows that the effectivity splint is more apparent compared to other therapies. According to Okeson 12 , the function of splint is to reduce muscle activities caused by parafunction while Kaplan and Assael 13 wrote about the functions of occlusal splint that include eliminating occlusion disorder, stabilizing teeth and joint relationship, relaxing muscles, eliminating parafunction, protecting teeth from abrassion and reducing TMJ load. The TMJ dysfunction patients who visit the Prosthodontic Specialist Clinic of Faculty of Dentistry, UNPAD, receive treatment using occlusal splint to eliminate the complaints. Okeson 12 wrote that the effectivity of splint in reducing TMJ dysfunction symptoms is 70-90%.

Abstrak — Bongkar-muat merupakan sebuah proses pemindahan suatu barang dari kendaraan pengangkut menuju gudang maupun sebaliknya. Pada sektor pergudangan terutama, dalam proses bongkar-muat masih dilakukan dengan cara konvensional. Hal tersebut menjadi kurang efisien dalam penggunaan tenaga kerja karena barang yang dipindahkan jumlahnya tidak sedikit terlebih jika barang tersebut berat. Dari permasalahan diatas untuk mengurangi beban kerja maka dibuatlah sebuah sistem bongkar-muat menggunakan lengan robot untuk proses pengambilan dan peletakan barang. Lengan robot yang terdiri dari base joint, shoulder joint, elbow joint, wrist joint dan vacuum gripper ini menerapkan metode back propagation neural network untuk menentukan sudut pergerakan motor servo pada tiap sendi terhadap koordinat pixel x, y yang dideteksi oleh kamera dengan penentuan lokasi kardus melalui proses klik operator pada hasil capture kamera pada proses bongkar. Hasil pelatihan back propagation pada lengan robot saat proses bongkar muatan mampu melakukan prediksi sudut pergerakan lengan robot dengan baik. Dengan 15 kali percobaan pergerakan lengan robot didapatkan error rata-rata sebesar1.588% pada koordinat x dan 1.982% pada koordinat y. Sehingga lengan robot mampu menuju kardus dan memindahkannya ke atas konveyor dengan baik. Pada penelitian ini dengan muatan sebanyak 24 kardus memiliki tingkat keberhasilan sebesar 100% pada proses bongkar dan 83.33% pada proses muat.
